A sound risk assessment starts with systematic hazard identification — walking the actual work sequence with the crew rather than assessing from a desk, and covering routine tasks as well as one-off, high-risk activities like lifting, hot work and confined space entry. Each hazard is rated for likelihood and severity to set priority, then controlled through the hierarchy of controls: eliminate or substitute the hazard where possible, apply engineering controls (guarding, ventilation, barricading), reinforce with administrative controls (procedures, permits, signage, training) and use PPE as the last line of defence, never the first. The assessment is only as good as its review cycle — it should be revisited whenever the scope of work, personnel, equipment or site conditions change, and signed off by the people actually doing the task, not just supervision. Documented, task-specific risk assessments (JSA/JHA) feeding directly into the Permit to Work system are what keep this from being a paperwork exercise and make it a working safety control.
A Permit to Work (PTW) system is the formal control that ensures high-risk activities — hot work, work at height, excavation, confined space entry and critical lifting — are only carried out once hazards have been assessed, controls are in place and the right people have signed off. An effective PTW isn't just a form: it requires a competent issuer and receiver who both physically verify site conditions before work starts, clearly stated validity periods and scope limits, cross-referencing to the relevant risk assessment and isolation/lock-out-tag-out requirements, and a live register so supervision always knows which high-risk activities are open on site at any given time. Permits should be closed out and the area re-inspected before normal operations resume, and any change in scope, personnel or conditions during the work should trigger a stop and re-permit rather than a verbal adjustment. Strong PTW discipline is one of the most effective single controls against major incidents on a construction or industrial site.
An emergency response plan is only effective if people know their role in it before an emergency happens. That starts with mapping realistic site-specific scenarios — fire, medical emergency, structural collapse, confined space rescue, chemical spill — rather than relying on a generic template, and assigning clear roles: incident controller, muster point marshals, first aiders and the liaison point for external emergency services. Assembly points, evacuation routes and firefighting/rescue equipment locations need to be clearly signed, kept clear of obstruction, and reviewed every time the site layout changes. Regular drills — not just fire drills, but scenario-specific ones such as confined space rescue or working-at-height rescue — are what actually test whether the plan works, and every drill should be followed by a debrief that feeds lessons back into the plan. Emergency contact numbers, the nearest medical facility and a clear communication chain should be posted at site entrances and reviewed at every induction, so that response time — not paperwork — is what the plan is judged on.
